Submit following documents, not less than 30 days before the date on which the licence expires: 

· Form No.4 (Application);

· Treasury challan (As a proof of Fees * ); 

- A duly stamped self addressed envelope to  receive the licence;

- Copy of electricity bill;

- Equipment test reports.

( * After expiry period, Late fees 25% additional is required.)

 

Upon submission of an application, the premises in respect of which such application is made shall be deemed to be duly licensed, until the Chief Inspector has passed an order either granting or refusing to grant or renewal or amendment or transfer of the licence.

Deemed to be duly licensed (Explanation) :

An application for the grant or renewal of a licence shall be deemed to have been duly made only if it is in the prescribed form (Form No.4) and is filled in with all relevant particulars and is accompanied by a treasury challan as proof of payment of the required fees.
